Case Manager

Reports To: School Principal

Position Summary:

The Civitas Case Manager serves as part of the Leadership Team and is responsible for ensuring that school programming supports growth and achievement for diverse learners. Case Managers lead diverse learner teams, ensuring that evidence-based and data-driven practices are implemented effectively and consistently. Case Managers partner with teachers to ensure that Individualized Education Programs and 504 Plans are implemented in compliance with federal, state, and local regulations and engage staff in creative, research-driven troubleshooting around academic and behavioral concerns. Case Managers also coordinate services for students based on the IEP and act to promote high levels of compliance with governing statutes. In addition, Case Managers work closely with the administrative team to advocate for diverse learners with respect to policy and procedures impacting students.

As members of the leadership team, case managers also assist the administration with tasks related to academic achievement, like intervention and enrichment programming, assessment development, curriculum analyses, professional development of teachers and paraprofessionals, etc.

Key Competencies/Duties:

Serves as the local school district representative at all required meetings under the IDEA and as the 504 Coordinator for all meetings under Section 504, fulfilling all related responsibilities and compliance requirements therein.

  • Ensures that all IDEA and Section 504 timelines are met.
  • Ensures that all parental notices and consents for evaluations and placements are processed and provided in the native language, when feasible, and placed in the student temporary files.
  • Ensures that parents/guardians receive a copy of draft IEP materials and, if applicable, paraprofessional, ESY ,and Learning Environment Intervention forms, along with copies of all other conducted evaluations and any other collected data at least five school days prior to the meeting.
  • Coordinates the assistance of an interpreter for IEP and 504 conferences (including initial and reevaluation eligibility determination meetings, and annual reviews) for parents who are limited English proficient.
  • Coordinates referrals for a Full and Individual Evaluation (FIE) to ensure that all required components are addressed and that the process is completed within 60 school days after receiving signed parental consent, including but not limited to, referrals for Assistive Technology/Alternative-Augmentative Communication evaluations or functional behavioral assessments (FBAs).
  • Convenes and chairs IEP and 504 conferences (including initial and reevaluation eligibility determination meetings, and annual reviews).
  • Ensures that IEP teams are utilizing qualitative and quantitative data when making service decisions in the development of an IEP.
  • Finalizes each student's IEP or 504 Plan in the SSM at the meeting and provides the student's parents/guardians with a copy.
  • Ensures that all relevant student data is entered into SSM in a timely manner.
  • Arranges special education and related services for a student within the school in accordance with the students IEP and communicates with the network Director of Diverse Learners regarding additional or modified services needed.
  • Consults with the Director of Diverse Learners regarding students who require low-incidence services, additional behavioral supports, or regarding problems that develop in the implementation of procedures.
  • In collaboration with the principal, oversees the implementation of all IEPs and 504 Plans for each student with disabilities in the school.
  • Trains and supports school staff regarding the implementation of the IDEA and Section 504 requirements.
  • Assists the school's principal or designee with programming for students with disabilities in accordance with their IEPs.
  • Works with the school nurse to ensure compliance with IEPs and 504 Plans.
  • Schedules paraprofessionals in accordance with students' IEPs and 504 Plans.
  • Coordinates and troubleshoots transportation for students with transportation services included in the IEP.
  • Supports special education teachers in writing cohesive, relevant, and purposeful IEPs and meeting administrative expectations and deadlines for assigned IEP caseloads.
  • Other duties as assigned.

Facilitates the intellectual and professional development of special education team members.

  • Engages teachers and paraprofessionals in reflecting on and shifting professional practices to generate academic progress and achievement for all students.
  • Produces measurable and observable improvements in teacher/paraprofessional performance.
  • Supports the instructional development of special education teachers in understanding and implementing universal designs for learning and differentiated instruction, assessment, and behavioral management strategies to promote student growth and achievement.
  • Assists teachers and paraprofessionals in understanding and fulfilling the school's vision, mission, and goals.
  • Provides technical support to collaborative teams within buildings.
  • Supports teachers and administrators in using data to improve instruction on all levels.
  • Contributes to the development systems and structures to improve teacher and paraprofessional practice within schools.

Focuses staff on outcomes, in keeping with the mission, vision, and goals of the school.

  • Leads meaningful data cycles for the special education team; monitors the application and effects of data-driven decisions.
  • Collects, analyzes, and evaluates data to determine programming strengths and areas for growth. Leads special education team through analyzing and applying data to shift practices.
  • Conducts crucial conversations based on student performance and survey data for the purpose of enhancing student learning and on-campus experiences.
  • Challenges directly practices that do not support the school's vision, mission, and goals.
  • Works to achieve metrics that support the school's needs.

Builds and maintains collaborative relationships.

  • Builds positive collaborative, cooperative relationships with other instructional coaches, administrators, staff, students, and parents.
  • Creates, develops, and sustains relationships that result in active teacher and student engagement in the learning process and the school community.
  • Collects and analyzes feedback of students in the evaluation of instructional, assessment, and relationship building practices and develops a course of action based on data.
  • Leads professional learning within schools and the network.
  • Leads with a balance of compassion and empathy, along with accountability and guidance.

Attends to and fulfills professional responsibilities as defined by the network, district, and state.

  • Leads with integrity and professionalism at all times, even in the face of conflict or ambiguity.

  • Provides temporary, short-term coverage of classes when necessary.
  • Takes on projects and initiatives as requested by the administration.
  • Attends summer training sessions.
  • Readily reflects on practices and shifts them to meet expectations or needs.
  • Acts as an ambassador, elevating the school's image and encouraging families to select and partner with the school.
  • Other duties as assigned.

  • Demonstrates preparation and skill in working with students from diverse cultural, economic and ability backgrounds.

Qualifying Education/Licensure:

  • Bachelor's Degree (Required)
  • Master's Degree or Commensurate experience leading diverse learner teams (Strongly Preferred)
  • Illinois State Board of Education Professional Educators License (PEL) (K-8; 6-12 or 9-12 - Required)
  • Endorsement as a Learning Behavior Specialist (LBS I/LBS II) or Director of Special Education (Required)
  • A minimum of 5 years of successful teaching experience in a elementary or secondary school (Required)
  • At least 1 year of experience leading teams and professional development for teachers (Strongly Preferred)
  • Bilingual Spanish/English; Arabic/English; Urdu/English (A Plus!)

Other Qualifications:

  • Experience supervising staff (Strongly Preferred)
  • Experience in urban school setting (Strongly Preferred)
  • Strong technical skills - ability to work with Google Suite, Microsoft Office Suite, presentation platforms and equipment, and student information databases, integrating these platforms when necessary (Required)
